Well it does not mean that you website hosting was under a "Hack Attempt". All CMS application have a security setting that does not allow the world to broswe the included PHP pages/codes. That is, all illegal browsing will through that message.

For example:
If you try to broswe:
http://domain.tld/cms/includes/
OR
http://domain.tld/cms/login_required/

will generate that message. It has nothing to do with the Hacking in any ways.

I think the problem is with either sessions or mod_security settings on the server in your case.
