Default What are the actual rules for nameservers?

I was wondering, what are the actual rules for nameservers? Some people say that they should be in IP pairs on the same class c, whereas others say they should be globally dispersed. Then there are others saying that both DNS servers should point to the same machine on different IPs. Will someone please tell me which combination for this is correct.
