Where have you been blacklisted? Spamhaus? Or some other RBL provider? You should have received an email about the abuse report and it should also contain information on how to get unlisted. I don't know what you mean about detecting the abuse report form, though. As for finding out who sent the abuse complaint, I'm afraid there's no way to do that.
