It all depends how you want to do it. If you want to create one which you update manually then all you need to do is create an xml file with the basic xml code required for a feed. You can also create dynamic feeds who's contents is made up from database information; to do this all you need to do is create a dynamic page (PHP, ASP etc) and enter the code which will connect you to the database and set the header content type to output the information as 'text/xml'; again, all you need to do after that is enter the revelevant xml code which will output a nested feed.
Here is an example of a basic RSS feed:
Code:
<?xml version="1.0"?>
<rss version="2.0">
<channel>

<title>Astronauts' Dirty Laundry</title>

<description>Compared to earlier spacecraft, the International Space Station has many luxuries, but laundry facilities are not one of them. Instead, astronauts have other options.</description>

The main feed is defined with it's own title and description tags, and each item is embedded with in an '<item>' tag and then has its own separate title and description tags to set the item out.

Aye she finally went 'live' last nite with three stories http://www.claytonnewmexico.net/rss/story.xml
I like how in Firefox it gives the option to add the RSS to your MY Yahoo and Google homepage too, as well as the live bookmarks. Tis a pity I have no idea how many have subscribed and whatever.

You can distribute your feed via FeedBurner; they let you see how many subscribers you have and things like that. Check it out: http://feedburner.com
